Hand pain from a keyboard and mouse comes from four specific tools, and each one produces a different problem. Buying a new device without knowing which tool is causing yours is how people end up with a drawer full of abandoned equipment and the same symptoms.

The mouse
A standard mouse holds your forearm rotated palm-down and asks the wrist to pivot on the desk. Two separate loads: the twist, and the clicking.
- Vertical mouse: fixes the twist. Good for forearm and wrist symptoms. Does nothing for click volume or grip force
- Trackball: removes arm movement entirely and moves the work into thumb or fingers. Excellent for some, and a direct route to thumb pain if your problem is already at the thumb base
- Larger mouse: if your hand claws to hold a small one, size up. Grip force falls immediately
- The real fix for click volume is shortcuts, not hardware. Every shortcut learned removes hundreds of clicks a week
The keyboard
- Split or tented keyboards reduce the outward wrist angle and forearm twist. The clearest benefit of any input device for ulnar-side wrist ache
- Key switch weight matters. Heavy switches mean more force per keystroke, thousands of times a day. Lighter switches are a genuine load reduction, not a preference
- Keyboard height and tilt. Tilting the back edge up pushes the wrist into extension. Flat or negatively tilted is kinder
- Number pad. A full-size keyboard pushes the mouse further right, which loads the shoulder and makes you reach on every movement. A tenkeyless board is a surprisingly effective change
Equipment changes where the load goes. Shortcuts and breaks change how much load there is. Only the second one has a ceiling worth chasing.
The wrist rest
The most misunderstood item on the desk.
It is meant to be a place to rest between bouts of typing, not a fulcrum to plant your wrists on while you work. Leaning the heel of the palm on a firm edge for hours compresses exactly the structures you are trying to protect, and contributes to both carpal tunnel syndrome and Guyon’s canal syndrome.
If you use one, it should be soft, and it should support the heel of the hand rather than the wrist crease.
The phone
Usually left out of the ergonomic assessment and often the largest single contributor, because the hours add to the same tissue rather than resetting.

Testing changes properly
- One change at a time, two weeks each. Five changes at once teaches you nothing
- Start with breaks, before any purchase. Thirty to sixty seconds off the desk every thirty minutes settles a meaningful share of early cases on its own
- Expect a worse first week with any new device. Tissue has to adapt to a new position, and people abandon good changes at day four
- If six weeks of sensible changes produces nothing, stop shopping and get the diagnosis confirmed
Match the tool to the symptom
- Numbness in thumb, index, middle: wrist position and rest habits. Carpal tunnel
- Numbness in ring and little fingers: pressure at the wrist or a bent, leaned-on elbow. Cubital tunnel
- Outer elbow pain gripping the mouse: tennis elbow, and it needs loading
- Thumb-side wrist pain: De Quervain’s, usually phone rather than keyboard
- Aching that builds and settles overnight: overuse, which equipment alone will not fix
