Swelling Is Not the Aftermath. It’s the Mechanism.
Swelling after a hand injury is the single biggest determinant of how stiff the hand ends up, because fluid sitting in and around the joints physically blocks movement and then turns into scar tissue that holds the joints where the fluid left them.
Most people treat it as cosmetic.
It isn’t.
The hand has almost no loose tissue on its palm side and a lot on its back, so swelling collects on the BACK of the hand and pulls the knuckles straight — the one position from which they stiffen fastest.
Why swelling stiffens a hand
Three mechanisms, running at once.
Volume. A joint capsule holds a fixed amount. Fill it with fluid and the joint cannot complete its range — the fluid is in the way.
Position. Fluid pushes the hand into what’s called the CLAW position: knuckles straight, fingers bent, thumb flat against the palm. Every ligament that shortens is one that resists you later.
Scar. Fluid sitting still is protein-rich. Left for weeks, it organizes into fibrous tissue, gluing tendons to their sheaths and capsules to bone.
Swelling that moves is fluid. Swelling that stays still for three weeks becomes architecture.
The safe position
This is the single most useful piece of information in the article, and it is why properly made splints look counterintuitive.
- Wrist extended around 20 to 30 degrees
- Knuckles (MCP joints) bent 70 to 90 degrees
- Finger joints (PIP and DIP) straight
- Thumb held out in opposition, not flat against the palm
In this position, the collateral ligaments of every joint sit at their LONGEST. If they shorten there, you lose almost nothing.
Immobilize a swollen hand with the knuckles straight — the way it naturally wants to sit, and the way a poorly applied bandage leaves it — and the same ligaments shorten in their slackest position. That hand does not make a fist again without months of work.
Anyone who has seen a hand splinted flat like a paddle for three weeks has seen where the next six months of therapy comes from.
What actually reduces it
Elevation. Above heart level. Not resting on a cushion at chest height, which does nothing. Hand above elbow, elbow above heart.
Active movement. The best pump in the hand is the hand. Veins and lymphatics have no pump of their own and rely on muscle contraction.
- Full fist, then full open, 10 times, every hour you are awake
- Move every joint that isn’t immobilized — shoulder and elbow included, because a stiff shoulder from an arm in a sling is a real and separate problem
Compression. Self-adherent wrap applied from fingertip towards the palm, or a compression glove. Firm enough to be felt, never enough to change color.
Cold. Useful in the first 48 to 72 hours. Not directly on skin, 10 to 15 minutes at a time. After the first few days it stops helping and movement matters more.
Remove rings immediately. Before, not after. A ring on a swelling finger becomes a tourniquet and then a cutting-tool problem.
When swelling is a warning
- Swelling INCREASING after 72 hours rather than settling
- Disproportionate pain, tense tissue, pain on passive stretch — see compartment syndrome
- Redness, heat, fever — infection
- Shiny skin, color change, hair or nail changes, pain far beyond expectation — see CRPS
The window
- Day 0 to 3: elevation, compression, cold, and every joint you’re allowed to move
- Week 1 to 3: the fluid is still mobile. This is the window where effort converts directly into range
- Week 3 to 6: organization into scar begins; progress slows noticeably
- Month 3 onward: what’s left is a stiffness problem, not a swelling one, and needs a different approach entirely
The boring interventions are the ones that decide the outcome. Elevation is free, unglamorous, and beats most of what follows it.
