Weak Grip: Why Your Hand Strength Dropped

Grip strength drops for one of three reasons:

  1. it hurts to squeeze,
  2. the muscles have wasted, or
  3. the nerve driving them is not getting the signal through

Working out which of those you have decides everything, and there is a quick way to tell them apart at home.

Try to grip hard. If pain stops you, it is a pain problem. If there is no pain and the hand simply will not produce force, it is a nerve or muscle problem, and that one is more urgent.

Weak because it hurts

Much the commonest.

The muscle works, but the brain inhibits it because squeezing loads a painful structure. Once the structure settles, the strength returns.

  • Pain at the outer or inner elbow when you grip: tennis elbow or golfer’s elbow. Gripping loads the tendon at the elbow, which is why a mug is the classic complaint
  • Pain at the base of the thumb opening jars and turning keys: thumb base arthritis. Pinch fails before grip does
  • Pain on the little-finger side when you push up out of a chair: TFCC injury
  • A generally sore, stiff hand after any injury or cast: disuse plus stiffness, and strength is the last thing to come back

Weak with numbness

A nerve problem until proven otherwise, and the fingers involved tell you which nerve.

  • Thumb, index and middle fingers numb, thumb pad looking flatter than the other side, dropping things: carpal tunnel syndrome, and at the stage where waiting costs you. Compare both palms in good light
  • Ring and little fingers numb, weak pinch, difficulty crossing the fingers, a hollow appearing in the web space between thumb and index: cubital tunnel syndrome. The ulnar nerve drives most of the small muscles inside the hand, so this one costs dexterity and grip together
  • Numbness in both hands, or pain running down from the neck: the problem may be above the arm entirely

Weak with no pain and no numbness

The combination that gets dismissed and shouldn’t be. Weakness alone points at the purely motor nerve branches, and there are two tests you can do right now.

  • Make an OK sign. Thumb and index tip to tip, forming a round circle. If the circle collapses into a pinch with flat fingertips, that suggests the anterior interosseous branch
  • Forearm flat on a table, palm down, lift the fingers and wrist. If they will not lift, that suggests the posterior interosseous branch

Both are covered in AIN and PIN palsy. Either result needs assessing rather than exercising.

Weakness with pain is usually the pain talking. Weakness without pain is the one to take seriously.

A note on measuring it

Compare sides rather than chasing a number.

A rough guide is that the dominant hand is around ten percent stronger, though this is variable and plenty of people break the rule. What matters is the direction of travel: if the gap between your hands is widening month to month, that is meaningful regardless of the absolute figures.

Also worth knowing: grip strength declines gradually with age in everyone. A slow change over years is different from a noticeable change over weeks.

Rebuilding it, once the cause is handled

  • Treat the structure first. Strengthening into a painful tendon or a compressed nerve makes both worse
  • Range before strength. A stiff hand cannot grip well regardless of muscle. Full fist and full extension daily
  • Then load progressively. Soft putty or a light ball, short sessions several times a day, building over weeks
  • Do not start with grippers. Spring grippers are usually far heavier than a recovering hand should be lifting

Get it assessed rather than trained if

  • You can see muscle wasting — a flattened thumb pad, or a hollow between thumb and index
  • The weakness came on over days rather than months
  • Numbness has become constant rather than intermittent
  • You are dropping things without noticing you were about to

Visible wasting means the nerve has been under pressure long enough to affect the muscle, and that recovers slowly and incompletely. It is the clearest signal on this page.
